You can get the forms from the New Orleans *Bureau* of Revenue, and
apply for a Louisiana tax id at the Louisiana *Department* of Revenue.
 I do not believe that one needs a Louisiana tax id in order to pay
sales tax here -- just a federal tax id or UBI# to put on the forms.
