Ray's method is correct and essentially answers Mr. Busbee's question.
Jeff's query is different. Being the new guy I'll take a stab at that.
In principle all you do is decide what parts you want and assemble it.
Ray's example is a quick and easy way.
If you were interested in perfect you would have to go a little further
setup a partition, compile custom kernel and setup the whole thing
exactly the way you want it then burn it.
This is essentially the way all Linux used to be prior to "distros".
There used to be a how to called something like "setting Linux up from
scratch" that would cover all but burning the image.
There are some hints in your request that may make this harder.
For instance even though there are browsers for the Internet that don't
work with X, I am fair certain that Flash will not be supported by
them. They are text based and are very fast because they ignore all but
text and links.
Second the desire to print.
If you are moving from machine to machine with this it may be rough to
predict what printers you will run into.
I have not looked at knoppix or any other of the "tiny" distros to see
how or if they addressed this issue.
I am certain some amount or type of printers could be supported but the
variety could not be limitless.
