I use spamassasin along with exiscan, exim and H+BEDV antivirus. All
incoming mails are checked for viruses and spam together and it works like a
charm. Of course if you're doing this as a sysadmin you'll want to check
outbound mail as well for viruses.

If you need an open source antivirus clamav is probably the best one.

There's a whole lot of antivirus - MTA stuff out there, it's just somewhat
of a pain to set up.
