Did you install clamav in a non-standard place? You could try configure
with --enable-clamd=/path/to/clamd

I installed clamav after i'd had mimedefang running for a while, and
simply added this to get mimedefang to start using clam:

$ClamdSock="/sendmail/mqueue/MIMEDefang/clamd.sock";
$Features{"Virus:CLAMD"}="/sendmail/software/sbin/clamd";
